Position Overview

The Senior Integration Engineer will own the technical and business relationship with national and regional payer and EHR partners, building event-driven integrations that move medication data, formulary updates, and prior authorization signals in near real time across the DrFirst platform.

 

This is a hands-on senior individual contributor role for an engineer who treats integration as a product, not a project queue. You will design Kafka-backed and FHIR subscription-based data flows, embed AI and machine learning into how we triage payer feeds and surface PA outliers, and serve as the technical authority Integration Services Engineers escalate to when payer logic gets messy.

 

You will partner with Product Management to convert one-off payer builds into repeatable platform capabilities, and you will help Anthropic-powered Claude workflows inside DrFirst classify payer payloads, draft remediation steps, and accelerate root cause analysis. What you will work on

Event-Driven Payer Integration

  • Design and build event-driven integrations between DrFirst and payer partners using Kafka, FHIR subscriptions, and webhook-based patterns
  • Architect streaming data flows for formulary updates, eligibility changes, and prior authorization events that scale across UnitedHealthcare, Aetna, Cigna, Humana, and BCBS plans
  • Replace batch-oriented payer feeds with near real-time pipelines where the data model and SLAs support it
  • Establish observability standards for payer event flows, including consumer lag, schema drift, and replay strategies

AI and ML in the Integration Workflow

  • Build and refine AI-assisted tooling that classifies payer payloads, flags anomalies, and drafts root cause analyses for integration incidents
  • Use Claude and similar large language models inside your daily workflow for code generation, schema mapping drafts, payer specification analysis, and runbook authoring
  • Partner with Product and Data Science teams on ML models that score prior authorization likelihood and surface formulary patterns that affect prescription orchestration
  • Document and share AI-augmented integration patterns so the broader Integration Services team adopts them

Payer Relationship Ownership

  • Serve as the primary technical and business contact for assigned payer partners across discovery, design, build, and steady-state operations
  • Run recurring technical syncs with payer counterparts, drive issue resolution, and translate payer constraints into actionable engineering decisions
  • Negotiate integration scope, data element changes, and timeline shifts with payer stakeholders while keeping internal product and delivery teams aligned

Prior Authorization Integration Depth

  • Build, maintain, and continuously optimize prior authorization integrations across direct payer connections and intermediary networks
  • Diagnose complex PA failures spanning eligibility, formulary, clinical criteria, and submission status feedback loops
  • Collaborate with clinical and product stakeholders to lift authorization success rates and reduce prescriber friction

Technical Leadership and Mentorship

  • Act as escalation point and technical mentor for Integration Services Engineers working on EMR, EHR, and payer integrations
  • Lead architecture reviews, code reviews, and integration design sessions that raise the technical bar across the team
  • Author internal Standard Operating Procedures, integration runbooks, and reusable code patterns that compress onboarding and reduce rework

Product Partnership and Platform Thinking

  • Partner with Product Management to convert custom payer builds into scalable platform capabilities exposed across DrFirst solutions
  • Influence the payer connectivity roadmap with data on integration cost, time to onboard, and incident rates
  • Own end-to-end delivery of integration projects from discovery through go-live, including dependencies, stakeholder communication, and post-launch monitoring

Qualifications

  • Bachelor's degree in Computer Science, Software Engineering, or a related technical field
  • 5 plus years in healthcare integration engineering with direct payer connectivity experience
  • Production experience with event-driven architectures including Kafka, RabbitMQ, Kinesis, FHIR subscriptions, or equivalent streaming and pub/sub patterns
  • Active daily use of AI tools (Claude, ChatGPT, Copilot, or equivalent) inside engineering workflows, including code generation, specification analysis, and documentation
  • Deep working knowledge of healthcare data standards including HL7 FHIR, HL7 v2, X12 EDI, and NCPDP
  • Proven track record managing external technical and business relationships with healthcare organizations
  • Hands-on development experience in Java, C#, Python, or JavaScript/Node.js
  • Working knowledge of CI/CD, automated testing, and AWS cloud services
  • Strong grasp of HIPAA, HITECH, and healthcare regulatory requirements as they apply to integration design

Preferred

  • Direct integration experience with national payer organizations or major PBMs
  • Experience applying machine learning models or LLM-based classification inside integration, data quality, or healthcare data pipelines
  • Background mentoring or leading integration engineers in a technical lead capacity
  • Familiarity with EMR and EHR integration patterns including Epic, Cerner, and Athenahealth
  • Working knowledge of microservices and event sourcing patterns at scale
  • Awareness of healthcare interoperability initiatives including TEFCA, CMS interoperability rules, and Da Vinci implementation guides

Physical Requirements

  • 90% Desk / work from home
  • 10% travel to partner sites, conferences, or internal team meetings

  • Competitive compensation, with a base salary of $120,000 - $140,000 (Exact compensation may vary based on skills and experience)
