Job Description: The Social Worker – BSW Senior, reporting to the Manager or designate, will coordinate care for patients, clients and residents. The incumbent will provide case management, education, advocacy, urgent consultations, referrals, and consultation to appropriate services and provides on-site and virtual social work services. “Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement”

Experience
- 2 years SW experience working with issues of capacity, dementia, grief, and loss
Education
- Bachelor of Social Work (BSW)
Licenses
- Valid Class 5 driver's license
